Besides being the evil queen and the mayor she also makes sure to ruin David and Mary Margaret's happiness as much as possible. As well as keep Henry and Emma separated when she can.

Storybrooke History:
When Emma Swan showed up at her door returning Henry she learned Henry had gone searching for his birth mother. When Henry went missing she went searching for him along with Emma. They questioned Mary Margaret who realized Henry had stolen her credit card to find her after she'd given him a book of fairy tales. Regina left and was later reunited with her son when Emma returned to him again. When Emma questioned if she loved him she said yes and Emma decided to stay convinced she lied to her.
Regina kept trying to get Emma into trouble and had her arrested for stealing Henry's files from Archie. But her plan backfired when Henry believed Emma's side of the story over her. She then was revealed to have found David/Charming and brought him to the hospital. When he woke up from his coma after Mary Margaret read to him she told Sheriff Graham to find him. With help from Emma, Mary Margaret, and Henry he did. Regina then showed up with David's wife Kathryn where they reunited. But David didn't remember her. He had fallen for Mary Margaret and they started dating.
Regina and Graham were revealed to have been dating. Graham dumped her having gained feelings for Emma who did in return. He talked to Henry and began to realize he was the Huntsman and had memory flashes of Fairy Tale Land. He went to Regina's father's crypt to find his heart. But Regina discovered them. She later revealed her chamber of hearts was beneath her father's coffin and took out Graham's heart and crushed it. Graham meanwhile was kissing Emma and remembered his entire life before dying of a heart attack since Regina killed him. Emma became the new sheriff against Regina's wishes to make Sydney the new sheriff.
When Regina discovered David & Mary Margaret's affair she tried to break them up, even took photos. When David broke up with Kathryn, Regina showed her the pictures. Furious she yelled at Regina and later Mary Margaret. But afterwards apologized to Regina saying she was just upset. She told her she left a letter for David and was planning to leave town. On the way out of town she vanished. Regina stole the letter she left to David and burned it. David was questioned for Kathryn's disappearance/murder and after Kathryn's heart was found in a box in a jewelry box with Mary Margaret's finger prints she was arrested.
While visiting Mary Margaret Regina let it slip she knew she was innocent. Henry stole a set of keys from Regina and discovered she had a key to Mary Margaret's home. Regina was annoyed Mary Margaret had returned with Emma's help. She later talkd with Rumple and they revealed they made a deal to release Rumple from jail and in return put Mary Margaret in jail.
Kathryn was found wandering around town and Mary Margaret was released. Regina confronted Rumple who revealed he never agreed to kill her. Regina framed Sydney for the job saying he wanted to be a hero and he was locked away in the asylum.
Regina later teamed up with Jefferson to use his hat using the last bit of magic she had including Daniel's ring to steal the poison apple she used on Snow White to use on Emma. She cut it up and made it into apple turnovers giving them to Emma who had decided to leave town and come back to visit Henry once and awhile. When Henry learned of the turnovers he ate one passing out. Emma rushed him to the hospital and soon realized it was all true. That Regina was the evil queen and had cursed the entire town. Regina admitted everything and they went to Mr. Gold/Rumple who told him he had a potion of true love he'd arranged to be brought with them. He told them it was inside Maleficent's body.
Emma went down alone while Regina operated the elevator and presumably killed Maleficent freeing the bottle. She went up only for Rumple to doublecross them and steal the bottle. Emma freed a tied up Regina and they returned to the hopsital to watch Henry die. Emma kissed him and woke him up, breaking the spell on him. She also reversed the curse restoring everyone's memories in the town. At the same time Rumple was reunited with Belle who had been freed from the asylum where Regina was holding her by Jefferson. Rumple activated his true love potion in the wishing well bringing magic to Storybrooke.
Fantasy History:
Regina has a close relationship with her father but her mother was a different story. Her mother had magic while Regina didn't originally. She wanted Regina to marry someone so they'd stay rich. Regina had fallen for a stable boy Daniel and they planed to elope. But when she saved Princess Snow White from being killed after falling off a horse her father proposed to Regina asking her to be his wife and a mother to Snow. She accepted reluctantly at her mother's urging. She later told Daniel and they kissed. Snow saw what was going on and left hurt.
Regina followed her telling her the truth and made her promise not to say anything. Snow broke her promise to Cora who killed Daniel in front of Regina and told her to mary the king. She reluctantly agreed heartbroken. When she learned Snow had broke her promise she was horrifed but hid it behind a mask.
She married the king and fell in love with the king's new friend Genie. With Genie and her father's help she killed the king using an Agravah Viper. Genie used the last wish of the lamp to always be by Regina's side turning himself into the Magic Mirror. She recruited a huntsman to kill Snow White in exchange for banning hunting of wolves. When the huntsman returned with a letter from snow and the heart of a stag she was furious and took the huntsman's heart instead and took him as her slave telling her guards to take him to her bedroom.
She later put out posters for Snow White's arrest. She met Belle when she left Rumple's castle telling her true love's kiss could break any curse. She then met Rumple after he'd thrown her out lying to him saying Belle had been shunned when she returned home and and killed herself. In reality she'd locked Belle away until she could find a way to use her against Rumplestilskin.
She gained the curse from Rumplestiltskin but decided not to use it. She traded it to Maleficent for her sleeping curse. Upon using it the apple was stolen by the Blind witch. She had Hansel and Gretel steal it from her and burned the Blind Witch after Hansel and Gretel had thrown her in the oven. When they handed the apple to her she decided to adopt them. They refused saying they wanted to be with theri father. She sent them into the woods so they could find him and later sent their father away escorted by his guards saying they could reunite if they found each other.
She reunited with Jefferson asking him to help her retrieve something the Queen of Hearts stole from her. At first he refused. She disguised herself as an old woman and refused to sell him a toy rabbit for a few copper coins he had. He later showed up at her castle agreeing to help her sending his daughter away to live with neighbors until he returned. They went into Wonderland and retrieved a box. Regina took some mushrooms and shoved it into the box which restored her father to normal size. Jefferson realized she was using him to free her father and the magic of the hat meant only two people could return from where they came. Regina trapped him and left with her father.
She was nearly killed by Snow White after she drank a potion to wipe her memories. But Charming helped her remember everything. She later took Charming from his "father" and set a trap for Snow. Snow took the apple thinking Regina would release Charming. The dwarfs, Red, and Granny found her and put her in a glass coffin. When Charming escaped Regina's castle he kissed Snow and woke her up. Furious Regina showed up at their wedding saying she'd cast a curse to ruin all their happpiness. She then went to Maleficent where they fought over the curse. She defeated her and stole the curse.
She then tried to cast the spell by killing her favorite horse. It failed. She then went to talk with Rumplestiltskin and learned she had to kill her father to make it work. She reluctantly killed her father and cast the spell banishing everyone to Storybrooke except for Pinocchio and Emma who escaped through an enchanted wardrobe.

Regina frowned as Henry left to go to his latest session with Archie. He'd been pulling away from her more and more. And he was spending a lot more time with Emma than she liked. And now that David and Mary where having an affair it infuriateed her. They weren't supposed to be together. They weren't supposed to get a second chance. She hadn't gotten her second chance with Daniel. And neither would they. She opened a drawer revealing photos she and Sydney had taken of David and Mary Margaret together. She'd make sure Kathryn saw these. She should have shown them earlier and now she would correct that mistake. She glanced out her window seeing Kathryn arrive. It was show time. Soon everyone would know about the affair and David and Mary Margaret's happiness would be destroyed.
